Worker Dara Phin, 45, Dies at Pacific Veneer Mill in Aberdeen, Washington

Dara Phin, 45, of Aberdeen, Washington died July 30 after being injured while operating machinery at Pacific Veneer, a wood products mill in Aberdeen, local authorities confirmed.

Coworkers and Aberdeen fire personnel performed life-saving measures, but Phin succumbed to his injuries and was declared deceased, The Daily World reported.

The Aberdeen Police Department, Aberdeen Fire Department, and the Grays Harbor County Coroner's Office responded to the incident.

Investigation

The Grays Harbor County Coroner's Office and Aberdeen police are investigating the circumstances of the death. Washington runs its own OSHA-approved safety agency — the Division of Occupational Safety and Health within the state Department of Labor and Industries — which has jurisdiction over workplace fatalities at private employers in the state. State and federal safety agencies typically take several months to complete a fatality investigation before issuing any citations.
