411 Press will update this report as the OSHA investigation develops. State and federal OSHA agencies typically take several months to complete a fatality investigation before issuing any citations or findings. The death is part of 411 Press's ongoing industrial accident coverage, which includes a worker killed by a rock crusher in Swampscott, Massachusetts and a worker crushed by a steel spool dropped from an overhead crane in Carol Stream, Illinois.
